You only have to complete this Declaration once and then just include your Gift Aid code for all of your future giving, so that we can match your giving to your Declaration. To create your Gift aid code, simply use two initials and then the type of donation you are making towards(tithe, offering, missions). Eg: Jane Smith giving towards tithes will be JS-tithe.


Confirm Gift Aid Declaration

I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax for each tax year (6 April to 5 April) that is at least equal to the amount of tax that all the charities and Community Amateur Sports Clubs (CASCs) that I donate to will reclaim on my gifts for that tax year. I understand that other taxes such as VAT and Council Tax do not qualify.

By typing your name below you confirm that you are a UK taxpayer and understand that if you pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all your donations in that tax year, it is your responsibility to pay any difference. You understand CityLife International will reclaim 25p of tax, depending on which box you ticked, on every £1 that you give.

You can cancel this Declaration any time by notifying the charity. Please notify the charity if you change your name or address or if you no longer pay sufficient tax on your income and/or capital gains. If you pay Income Tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self Assessment tax return or ask HM Revenue & Customs to adjust your tax code.
